Whitney Young Health Partners with NuLink Health to Launch Innovative Hybrid Care Program for Underserved Communities
Albany, NY – August. 27, 2025 — NuLink Health and Whitney M. Young Jr. Health Center are proud to announce a strategic partnership to launch a new hybrid care service line designed to meet patients where they are—beyond and between traditional office visits. This initiative will integrate NuLink’s Remote Physiological Monitoring (RPM) and Care Management services into Whitney Young Health’s clinical operations, delivering high-touch, technology-enabled support for patients managing chronic conditions such as hypertension.
Established in 1971, Whitney Young Health is a nonprofit Federally Qualified Health Center (FQHC) serving the Capital Region through its comprehensive network of medical, dental, behavioral health, school-based, and mobile health services. This partnership marks a bold step in Whitney Young Health’s mission to expand access, improve care coordination, and enhance the experience for both patients and providers.
NuLink Health, LLC was formed in 2022 as a partnership between health technology companies UTMHealthcare, LLC and Somml Health, LLC.

The new hybrid service line will leverage NuLink Health’s services to provide:
- Real-time Monitoring: Connected devices for blood pressure and other vital signs, integrated into the patient’s electronic health record
- Clinical Escalation Pathways: Protocols aligned with provider-defined thresholds, guidelines, clinical workflows and individual patient care goals.
- Proactive Engagement: Daily/Weekly patient check-ins, technical support, and education to promote self-management, adherence and proactive management.
- Care Coordination: Communication between NuLink Health, Whitney Young clinical teams to address risk, escalate urgent concerns, and track outcomes.

The collaboration will initially focus on patients with hypertension, with plans to expand to other chronic and high-risk conditions. Patients will be enrolled in the program by their primary care providers and supported by care managers and nurses trained in chronic disease management.
By embedding virtual care into Whitney Young’s existing infrastructure, the hybrid model will enhance provider efficiency, improve patient and provider satisfaction and enhance care and quality between visits.
